Body transformation may be considered a social determinant of health among transgender people. This article examines three challenges in ensuring universal access to the transexualizer process providedservices provided by the Brazilian National Health System (SUS). A qualitative study was conducted using audio-recorded narrative interviews. Current challenges include: 1. Poor geographical distribution of programs, with six out 10 of the country’s gender reassignment facilities located in the Southeast Region and absence of facilities in the North Region; 2. Discrimination and lack of respect in relation to preferred names, which hampers access to health services; 3. Diagnosis of transsexualism is guided by socially constructed gender norms, preventing access to transgender services.
