Abstracts

The aim of this experience report is to present the Rua na Rede (Street in the Network) device, created by the Consultório na Rua (eCR) team in Florianópolis – SC, as a strategy for building integrality and longitudinal care for eCR users during hospitalization and after discharge. The authors’ experience of building the device was analyzed through their field diary, in dialogue with the scientific literature on the subject. The report underlines that Rua na Rede is guided by the Harm Reduction paradigm and faces an important challenge for the Brazilian National Health System: the link between Primary Care and Hospital Care. A nodal point for integrality and longitudinal care for homeless people.
